One Survey Code per Cohort

A survey printed on one poster gives you one number: total responses. A survey code per cohort — per store, per session, per class — tells you where responses came from without asking respondents to identify themselves. This tool takes a table of surveys and cohorts and produces one code per row, each tagged with its cohort.

Keeping responses anonymous

Cohort tags describe groups, not people. If you add a respondent reference, make it a random code from your own list, not a name, email, or employee number. Respondents answer more honestly when the link cannot identify them, and you stay on the right side of privacy rules.

Before you print the batch

  • Scan a sample code per survey and confirm it opens the right survey.
  • Confirm each cohort tag lands in the survey tool as expected.
  • Print the survey name next to each code so staff place the right poster in the right place.
  • Keep the source list with the job file so responses can be matched to cohorts.

Frequently Asked Questions

What format do the rows need?

One row per line, comma-separated, or with the | separator. The first row is treated as a header and skipped when it looks like a header row. Rows that are missing a required field are reported and skipped without blocking the valid rows.

Can I use a spreadsheet?

Yes. Export the sheet as CSV, then paste the CSV text here. Keep one record per row and keep the column order shown above the input.

How many rows can I generate at once?

Up to 500 rows per batch. Split larger lists into multiple batches and label each ZIP.

Does my data leave my browser?

No. Every QR code is generated locally in your browser. Nothing is uploaded.
